Get health coverage at low or no cost to you

Individuals and families who can’t get any other coverage can apply for the Community Health Access Program (CHAP) from Kaiser Permanente.

What is the Community Health Access Program?

The Community Health Access Program (CHAP) provides health coverage for individuals and families who don’t have access to other health coverage. CHAP members pay no monthly premiums and have no out-of-pocket costs for most care at Kaiser Permanente facilities.

Who is eligible for CHAP?

You or your family may qualify for CHAP if you meet these requirements:

    • Live in the Kaiser Foundation Health Plan of the Mid-Atlantic States, Inc. service area, excluding the District of Columbia
    • Your household income is no more than 3 times the federal poverty level (for example, up to $38,640 for a single person or $79,500 for a family of 4 in 2021)
    • You aren’t eligible for any other health coverage, like Medicaid, Maryland Children’s Health Program, FAMIS, Medicare, a job-based health plan, or financial help through the Maryland Health Connection or the health benefit exchange

You don’t need to be U.S. citizens to qualify for the Community Health Access Program.

Applying for CHAP

Community Health Access Program members whose subsidy will expire on December 31, 2022, should have received reapplication information. You MUST provide Kaiser Permanente the signed form, proof of household income, and other applicable documents by October 1, 2022, to determine if you still qualify for financial help in 2023.
