You can apply for CHCP during the open enrollment period, which begins November 1, 2026 to January 15, 2027.

To apply for CHCP, you must work with a CHCP community partner.

Submitted applications are processed in order of receipt. We’ll let you know if you’ve been approved for CHCP after we receive and review your completed forms and any proof of income. Please allow up to 6 weeks for processing.
