With Community Health Coverage Progam (CHCP), you are enrolled in Kaiser Permanente’s MD Gold 0 Ded/150Rx Ded/Vision plan or KP VA Gold 0 Ded/Vision plan. These plans include many benefits, such as preventive care, medication, vision services, and pediatric dental coverage.

Members can also get a paper copy at any time without charge by calling Member Services at 1-800-777-7902. You’ll need your plan name, which is stated above.

Members can also review the Combined Membership Agreement, Disclosure Form, and Evidence of Coverage for the Kaiser Permanente MD Gold 0 Ded/150Rx Ded/Vision plan or KP VA Gold 0 Ded/Vision plan, which provides detailed plan information.

Note: These documents show copayments for services under the Kaiser Permanente’s MD Gold 0 Ded/150Rx Ded/Vision plan or KP VA Gold 0 Ded/Vision plan. Under Community Health Access Program, you do not have to pay co-payments for most covered services at Kaiser Permanente’s medical offices.


Special Enrollment Period

Qualifying Life Event (QLE)

A change in your situation — adding a dependent, such as getting married, having a baby, or adoption of a child —  can make you eligible for a Special Enrollment Period, allowing you to enroll into the CHCP outside the Open Enrollment Period.

Add a family member

Kaiser Permanente CHCP members who want to add an eligible family member to their existing account should use the Account Change Form.

  • Consider Medicaid.This option may be available if you were born in the United States, you are a legal permanent resident, and your yearly income is at or below 138% of the federal poverty level ($17,609 for an individual or
    $36,156 for a family of 4 in 2020). Kaiser Permanente is a Medicaid provider and may be available to you. Please visit kp.org/medicaid/md for more information.
  • Buy health coverage through Maryland Health Connection. If you qualify, you may get help paying for your plan premiums or out-of-pocket costs. For more information visit marylandhealthconnection.gov.
  • Call the Kaiser Permanente National Consumer Sales Center at 1-800-488-3590 or visit buykp.org to learn about other Kaiser Permanente Individuals and Families plan choices.
VA
