Qualify for Community Health Care Program

You or your family may qualify if you live in a Kaiser Permanente service area and meet these age requirements:

  • Fresno, Kings, or Madera counties: under 26
  • Other counties in Northern California: 18 or younger

You must also:

  • Live in a household with an income no more than 3 times the federal poverty level (for example, up to $36,420 for a single person or $75,300 for a family of 4 in 2018)
  • Not have access to other health coverage like Medi-Cal, Medicare, a job-based health plan, or coverage through Covered California

You don’t need to be U.S. citizens to qualify for the Community Health Care Program.

Please note: the program is open in all Kaiser Permanente Northern California service areas except Santa Cruz county.

We review each member’s eligibility for the Community Health Care Program by regularly sending our members a form to confirm income and other information. If you stop meeting these eligibility requirements, you won’t be able to stay in the Community Health Care Program. Enrollment in the Kaiser Permanente Platinum 90 – HMO plan will continue, but you will be required to pay the full monthly premium and your out-of-pocket costs, unless you ask us to terminate your membership.

If you do not meet the requirements for the Community Health Care Program, financial help from the government could help you get affordable health coverage for you and your family. Other Kaiser Permanente health coverage options include:


Medi-Cal coverage is provided to all low income children under the age of 19, regardless of immigration status. Medi-Cal coverage offers a comprehensive set of benefits for children comparable to those available to Community Health Care Program members.

If your family’s total annual household income is less than 267% of the federal poverty level or $67,017 for a family of 4 per 2018 guidelines, you and your family may be eligible for Medi-Cal.

Medi-Cal enrollment is year-round.

Health insurance plans from Covered California

You can buy health care coverage on California’s Health Insurance Marketplace, Covered California. Covered California has Kaiser Permanente plans as well as other health plans for purchase. If you meet certain income guidelines, you may be able to get help paying for premiums and copays, but only if you purchase coverage through Covered California’s website.
