Reapply to remain in CHCP

CHCP Reapplication takes place from August 1 to October 1, 2026. Reapplication helps us determine if you are still eligible for the CHCP subsidy for the 2027 plan year.

Current CHCP members whose subsidy will expire on December 31, 2026 and who are eligible to reapply for the program, must reapply between August 1, 2026 and October 1, 2026 to continue receiving the CHCP subsidy in 2027. You MUST provide Kaiser Permanente the signed form, proof of household income, and other applicable documents by October 1, 2026 to determine if you still qualify for financial help in 2027.

Individuals over 21 are eligible to receive the CHCP subsidy for a maximum of up to 3 years in a row. Eligible dependents can continue to receive the subsidy until they turn 21, no matter how long they’ve been in the program. When a covered child turns 21, the 3-year limit starts.

If you don’t reapply, your CHCP subsidy will expire on December 31, 2026. The CHCP subsidy is available as a one-time resource and all current members must reapply annually. Members who do not reapply in consecutive years will lose future eligibility.

Reapply online

Current CHCP members can reapply online in a few easy steps. Click the button to get started. You will need the primary member’s Medical Record Number (MRN) and Date of Birth to confirm your eligibility to reapply for the CHCP subsidy.

Need CHCP Primary Member’s Medical Record Number?

  • Listed on the primary member’s KP membership card
  • Listed in CHCP Reapplication materials which were sent by U.S. mail in early August 2026
  • If you received help from a CHCP Community Partner when you first applied to the program, you can contact them for help.
  • You can also call Kaiser Permanente at 1-800-464-4000 (TTY 711), 24 hours a day, 7 days a week (except holidays).

You are enrolled in the Kaiser Permanente Platinum – 90 HMO health plan.

This plan covers preventive care, medication, pediatric dental coverage and vision services (up to age 19), and more. To make things easier, many of our locations have lab, X-ray, and pharmacy services all in one place. And if you can’t come in during the day, some facilities offer evening and weekend hours.

Monthly payment

There is no monthly payment required. Kaiser Permanente will subsidize the full monthly premium.  There are low or no costs for most covered services at Kaiser Permanente facilities.

Act for a family member

If you have your own kp.org member account, you can help certain family members with their health care by making appointments, looking over most of their test results, emailing their doctor, and more. For more information, go to Act for a Family Member.

If you do not have a kp.org member account and would like to act on behalf of someone who is a Kaiser Permanente member, you can set up a non-member caregiver account. Set up your account here.

Making changes to your account

CHCP members can use the Account Change Form to make changes to their account, including name changes, address changes, and requests to end health coverage.

Important documents

Note: These documents show copayments for services under the Kaiser Permanente Platinum 90 – HMO plan. With CHCP, you don’t have to pay out-of-pocket costs for most covered services at Kaiser Permanente’s medical offices and hospitals.

* These features are available only for care at Kaiser Permanente facilities. Due to privacy laws, certain features may not be available when they are being accessed on behalf of a child 18 or younger, and your child’s physician may be prevented from disclosing certain information to you without your child’s consent.
