Case management
What is case management?
Case Management is a collaborative process that assess, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s health needs, using communication and available resources to promote quality effective outcomes.
If someone has a health condition and needs help with managing their chronic conditions, such as asthma, cancer, heart disease, or diabetes, please click on the: case management referral flyer for more information. If you are leaving the CHCP on December 31, the member should contact case management and state they are seeking transition case management.
