Participating Community Partners

To assist in the identification and eligibility determination of potential members, Kaiser Permanente partners with local organizations, schools and workforce development agencies. All community partners provide valuable services to their clients and students to guide them with next steps beyond the Kaiser Permanente Bridge Program. In order to qualify for the Bridge Program, those interested in applying must be actively enrolled with a participating community partner along with additional eligibility criteria.

Interested in becoming a community partner? We are no longer accepting applications for the 2025 Enrollment Period. Please complete and submit the Bridge Program Community Partner Application by Wednesday, April 30, 2025 to be considered for the 2026 Enrollment Period.

Below is a list of our participating community partners, their contact information, and information about the services they provide. If you are interested in applying for the Bridge Program, please contact the community partner. No enrollment is done directly through Kaiser Permanente.

The Atlanta Children’s Shelter is dedicated to helping families overcome the issues that contribute to homelessness, including domestic violence and job loss. The organization provides a total family service model exclusively to homeless families with young children under the age of 5. The focus is on the long-term self-sufficiency of the family through job skills training, emotional support, and child care.

The Atlanta Community Food Bank sources food for pantries to distribute. They also do direct food distribution (off site at our Community Food Centers) and direct application assistance for SNAP and Medical Assistance.

The Bridge Program is open only to students who reside in counties served by Kaiser Permanente and meet program eligibility requirements. The student must be affiliated with Chattahoochee Technical College at the time of application to the KP Bridge program with anticipated, continued enrollment until goal completion. Students must attend a one-on-one, virtual, or group application session. All information must be reviewed by qualified CTC staff prior to application submission.

  • Bachelor of Science in Nursing (accepted into the nursing program, not pre-nursing students)
  • Bachelor of Science in Dental Hygiene (accepted into the dental hygiene program, not pre-dental hygiene students)

East Lake Healthy Connections is a program of the East Lake Family YMCA. Healthy Connections has a team of Family Health Advocates whose mission is to promote increased education and access to primary and preventive health care services to the residents and students in the East Lake community. The Family Health Advocates seek to enroll all eligible adults and children in an insurance plan, help everyone identify and establish a medical home, teach proper ways to use a medical home to increase overall health, and provide education about chronic conditions such as asthma, diabetes, and hypertension. Healthy Connections offers the following programs: chronic disease prevention and management; healthy eating and physical activity; weekly food distribution; stress management; family planning service guidance; 504/IEP support; and housing resources (utility assistance in DeKalb County).

First Africa Community Development Corporation (FACDC) is a non-profit providing food pantry services and furnishing home services to residents throughout metro Atlanta. FACDC distributes over 2 million pounds of food each year through numerous food drives. The Kaiser Bridge Program services are offered to customers who are active participants in an approved training program in the Employment and Training Adult and Youth Units. Customers are eligible to receive benefits upon acceptance into one of the acceptable training programs. Customers must maintain consistent attendance in the required activity and comply with meeting identified goals in the individuals employment plan.
