Qualify for the Colorado Bridge Program
You or your family may be eligible for Colorado Bridge if you:
- Live in the Kaiser Foundation Health Plan of Colorado Denver/Boulder service area
- You must live in Adams, Arapahoe, Boulder, Broomfield, Denver, Douglas, or Jefferson County.
- Live in a household with an income up to 300% of the federal poverty level (for example, up to $45,180 for an individual or $93,600 for a family of 4 in 2024)
- Are not eligible for other public or private health coverage such as, but not limited to, Health First Colorado (Colorado’s Medicaid Program), OmniSalud, Medicare, Child Health Plan Plus (CHP+), a job-based health plan, or financial help through Connect for Health Colorado
- As of November 1, 2024 people with Deferred Action for Childhood Arrivals (DACA) status will be newly eligible for health insurance plans and financial help on Connect for Health Colorado.
- As of January 1, 2025 all children from households with incomes up to 265% of the federal poverty level ($39,909 for an individual or $82,680 for a family of 4 in 2024) are eligible for Health First Colorado/CHP+ regardless of immigration status.

You don’t need to be U.S. citizens to qualify for the Colorado Bridge Program.
Kaiser Permanente service areas
You must live in a Kaiser Permanente service area to qualify for the Colorado Bridge Program.
We review each member’s eligibility for the Colorado Bridge Program by regularly sending you a form to confirm household income and other information. If you no longer meet the eligibility requirements, you will be disenrolled from Kaiser Permanente’s Colorado Bridge Program. You will remain enrolled in the KP CO Gold 0/25 RX Copay plan, but you’ll have to pay the full monthly premiums and out-of-pocket costs unless you ask us to end your membership or until you fail to pay the full premium.

Health First Colorado (Colorado’s Medicaid Program)
You may be eligible for Health First Colorado (Colorado’s Medicaid Program) or Child Health Plan Plus (CHP+) if:

You’re 19 or older with a total household income at or below 138% of the federal poverty level (for example, $20,783 for a single person or $43,056 for a family of 4 in 2024).

You’re younger than 19 with a total household income at or below 265% of the federal poverty level (for example, $82,680 for a family of 4 in 2024).
Health coverage plans through Connect for Health Colorado
You can buy health care coverage on Colorado’s health benefit exchange, Connect for Health Colorado. If you qualify, you may get help paying for your plan premiums or out-of-pocket costs. Remember to enroll during Connect for Health Colorado’s open enrollment period. If you wait until after the open enrollment period ends, you’ll need a qualifying life event to enroll in a new plan.
