If you’re a Colorado resident without access to other health coverage, you may get high-quality health care through CHCP.
What is CHCP?
CHCP is for Colorado residents who can’t get any other health coverage. CHCP members receive a subsidy to help pay monthly premiums, and do not have to pay copays or out-of-pocket costs for most care at Kaiser Permanente facilities.

Who is eligible for CHCP?
You may qualify for CHCP if you live in the Kaiser Foundation Health Plan of Colorado service area and you:
- Live in Adams, Arapahoe, Boulder, Broomfield, Denver, Douglas, or Jefferson, or Weld County
- Live in a household with an income up to 300% of the federal poverty level (for example, up to $46,950 for an individual or $96,450 for a family of 4 in 2025)
- Are not eligible for other public or private health coverage such as, but not limited to, Health First Colorado (Colorado’s Medicaid Program), OmniSalud, Medicare, Child Health Plan Plus (CHP+), an affordable job-based health plan, or financial help through Connect for Health Colorado

Applying for CHCP
Current CHCP members whose subsidy will expire on December 31, 2025 and who are eligible to reapply for the program, must reapply between August 1, 2025 – October 1, 2025 to continue receiving the CHCP subsidy in 2026. You MUST provide Kaiser Permanente the signed form, proof of household income, and other applicable documents by October 1, 2025 to determine if you still qualify for financial help in 2026.
