Children and young adults who can’t get any other coverage can apply for the Community Health Care Program (CHCP) from Kaiser Permanente.

What is the Community Health Care Program?

The Community Health Care Program is for children and young adults who can’t get any other health coverage. Community Health Care Program members receive a subsidy to help pay monthly premiums, and do not have to pay copays or out-of-pocket costs for most care at Kaiser Permanente facilities.

Who is eligible for CHCP?

You’re eligible for CHCP if:

  • You live in a Kaiser Permanente service area.
    • You’re 25 and under and live in Fresno, Kings, or Madera counties
    • You’re 18 and under and live in another county in Northern California
  • Your household income is no more than 3 times the federal poverty level (for example, up to $37,470 for a single person or $77,250 for a family of 4 in 2019).
  • You don’t have access to any other health coverage, like Medi-Cal, Medicare, a job-based health plan, or coverage through Covered California.

You don’t need to be U.S. citizens to qualify for the Community Health Care Program.

Applying for CHCP

Open enrollment for 2020 has ended, but you can still apply during a special enrollment period if you experience a qualifying life event.

CHCP members whose subsidy will expire on December 31, 2020, should have received reapplication information. Members MUST provide Kaiser Permanente their signed form, proof of household income, and other applicable documents by October 1, 2020, to determine if you still qualify for financial help in 2021.
