Participating Community Partners<\/h1>\n

To assist in the identification and eligibility determination of potential members, Kaiser Permanente partners with local organizations, schools and workforce development agencies. All community partners provide valuable services to their clients and students to guide them with next steps beyond the Kaiser Permanente Community Health Coverage Program. In order to qualify for CHCP, those interested in applying must be actively enrolled with a participating community partner along with additional eligibility criteria<\/a>.<\/p>\n

The organization provides a total family service model exclusively to homeless families with young children under the age of 5.
