Get Help with the Community Health Coverage Program (CHCP)<\/h1>\n

Below is a list of our most frequently asked questions. Please review the answers to help address your needs. Be sure to provide the representative your name, date of birth, and the date you applied.<\/p>\n

NOTE:<\/strong>\u00a0Allow 6 weeks for your application to process<\/u>. You can apply for this program during the annual Open Enrollment Period, which is November 1 \u2013 January 15.<\/p>\n

Please note: <\/strong>If you are not able to keep your scheduled appointment time at one of our Kaiser Permanente facilities and do not give us 24-hour notice, you will be charged a $25 missed appointment fee by your physician’s office. For questions related to your CHCP membership ending, please review the following:<\/strong><\/h2>\n[\/vc_column_text][vc_tta_accordion spacing=”5″ gap=”5″ active_section=”” collapsible_all=”true”][vc_tta_section title=”Can I reapply for CHCP?” tab_id=”1545199906267-0e66c113-9501″][vc_column_text css=””]Yes, you may reapply for the Bridge Program during the Annual Reapplication Period (August 1 \u2013 October 1) if you still meet the program eligibility requirements. You will receive the Reapplication Packet in the mail in early August. You may also request a PDF version of the form from your community partner or email bridge@kp.org. Starting in 2026, there is a maximum subsidy duration of up to 3 years in a row.<\/p>\n

Please Note: <\/strong>You must continue to meet all the eligibility criteria for the program. You will need to obtain a community partner verification letter from your community partner and submit current income documents.[\/vc_column_text][\/vc_tta_section][vc_tta_section title=”What if I miss the Annual Reapplication Period?” tab_id=”1655836891537-609c067f-f915″][vc_column_text css=””]If you do not complete and submit the reapplication form by the October 1 deadline, you can still apply during the Open Enrollment Period (November 1 \u2013 January 15).
